Completing Your Forms
To save you time at your first visit, we take care of new patient paperwork ahead of time. Our office sends over a short Patient Registration and Smile Profile packet before your appointment, which you can fill out at home and bring with you, rather than filling out several pages in the waiting room. Most patients find it takes just a few quiet minutes to finish both forms.
If you were not able to complete your forms in advance, that is not a problem. Just arrive a few minutes early and our front-office team will help you get everything filled out before you are seen, so it does not cut into your appointment time. We would rather you take the time you need than rush through your medical history.
Completing your forms ahead of time also helps our team prepare for your visit. It gives Dr. Solomon a head start on understanding your dental history and any concerns you want to bring up, so more of your appointment can be spent on your actual care instead of paperwork.
What to Bring to Your Appointment
Along with your completed forms, please plan to bring the following items with you to your first appointment:
- A valid photo ID
- Your insurance card, if you have dental insurance
- A list of any medications you currently take, including dosage if you have it handy
- Your completed forms, if you were able to fill them out ahead of time

Before You Arrive
Want to know what happens once you arrive? Visit our what to expect page for a full walkthrough of your first appointment, from check-in through your treatment plan, and check our insurance page to confirm we work with your plan before your visit.
If you have any questions about the forms themselves, or anything else before your visit, our team is always happy to help over the phone. There is no such thing as a question too small to ask before your first appointment, and we would rather answer it in advance than have you worry about it on the drive over.
